紙の賞状を希望される受賞酒蔵様には、本フォームの申請をお願いしております。紙版の証明書は、事務所や酒蔵に掲示していただき、受賞の栄誉を広く発信するためのツールとしてご活用いただけます。なお、発行には料金がかかりますので、あらかじめご了承ください。

Congratulations on receiving an award at our Sake Challenge! We appreciate all your relentless efforts and beautiful work. Submission of this form is required if you are an awarded brewery that is willing to obtain a hard copy of the award certificate. You can make good use of our paper-based certificate to showcase your achievements by posting it in your office or brewery. Please note a fee applies for this service. Thank you for your understanding.

申請前に必ずご記入内容が正しいことを確かめてください。本フォームの内容に基づき証明書が発行されるため、証明書の内容に誤りがあった場合でも弊協会では一切の責任を負いません。また、本サービスは公式に受賞が認められた酒のみを対象としており、未認可または不正な申請は処理いたし兼ねますことをご了承ください。ご協力のほど、何卒よろしくお願い申し上げます。

Ensure all the information on this form is correct. The Sake Sommelier Association refers to the content on the form to issue a paper-based certificate. Even in case an error occurs in the certificate, we are unable to take any responsibility. Make sure you understand this service is valid only for the officially awarded sake, meaning unauthorised sake or inappropriate submissions will not be processed. 過去の酒チャレンジで既に紙版の証書を受け取られた受賞者の方で、デジタル版もご希望の場合は、お手数ですが下記のメールアドレスまでお問い合わせをお願いいたします。折り返し、お申込みフォームをお送りします。なお、発行には料金（£15）がかかりますので、あらかじめご了承ください。
